Efecto del Tratamiento Periodontal no Quirúrgico en los Niveles de MMP-2 y TIMP-2 en Periodontitis Crónica

Effect of Non-Surgical Periodontal Treatment in the Levels of MMP-2 and TIMP-2 in Chronic Periodontitis

Resumen

La MMP-2 es una gelatinasa envuelta en la degradación de la matriz extracelular ya sea directamente por sus efectos degradativos o por la capacidad de procesar sustratos bioactivos. El objetivo de este estudio fue determinar el efecto del tratamiento periodontal no quirúrgico en los niveles de MMP-2 y TIMP-2 en el fluído gingival crevicular (FGC) de sitios con periodontitis crónica.

Materiales y Métodos
Se estudiaron sitios de individuos con periodontitis crónica los cuales fueron seguidos hasta completar el tratamiento periodontal no quirúrgico. Se obtuvieron biopsias (pre-tratamiento) y FGC (pre y post tratamiento) de sitios con periodontitis crónica y de sitios sanos. Se observó la expresión de MMP-2 y TIMP-2 en tejidos gingivales enfermos y sanos mediante inmunohistoquímica y se determinaron sus niveles mediante la técnica ELISA.

Resultados
Observamos presencia de MMP-2 y TIMP-2 en biopsias de sitios con periodontitis (pre-tratamiento) y sitios sanos. Los niveles de MMP-2 fueron mayores en el FGC de individuos con periodontitis en comparación a individuos sanos. Los niveles de MMP-2 disminuyeron en los sitios afectados luego de realizado el tratamiento periodontal no quirúrgico. No encontramos diferencias significativas en los niveles de TIMP-2 al comparar sitios antes o después del tratamiento.

Conclusión
La MMP-2 podría tener un rol dentro de los fenómenos de destrucción tisular que caracteriza a la periodontitis cónica. El tratamiento periodontal no quirúrgico disminuye los niveles de MMP-2 en sitios con periodontitis crónica.

Abstract

Matrix metalloproteinase (MMP)-2 is a gelatinase involved in extracellular matrix degradation either by its direct degradative effects or by processing bioactive substrates. The aim of this study was to determine the effect of conventional tratment in the levels of MMP-2 and tissue inhibitor of metalloproteinase (TIMP)-2 in gingival crevicular fluid (GCF) of chronic periodontitis patients.

Materials and Methods
In this study chronic periodontitis patients with moderate to severe disease were followed until they complete the non-surgical periodontal theraphy. Gingival biopses were obteined from periodontitis (pre-treatment) and healty sites. GCF samples were obteined from periodontitis (pre and post treatment) and healthy sites. MMP-2 and TIMP-2 detection were carried out by immunihistochemistry and levels determination by enzyme-linked immunosorbent assay (ELISA).

Results
We observed the expression of MMP-2 y TIMP-2 in gingival biopses from periodontitis (pre-treatment) and healthy sites. The levels of MMP-2 were higher in GCF of disease sites in relation to healthy sites and lower post-treatment sites in comparation with pre-treatment sites. There were no significant diferences in the levels of TIMP-2 before or after nonsurgical periodontal treatment.

Conclusion
MMP-2 could play a role in tissue destruction a characteristic of chronic periodontitis. Nonsurgical periodontal treatment decrease the levels of MMP-2 in sites with chronic periodontitis.
